Yamhill County Fair

Essential Services will have a booth at the Yamhill County fair. Stop by to meet some of our Administrative team and get some new swag!

Entry to the fair is ticketed; Essential Services will have tickets for our current clients and DSPs at 536 NE Baker St beginning the week before the fair. You can pick up tickets during our regular business hours, M-F 9 am- 5 pm.

Please note: Rides and food are not included in the tickets; they are entry only.

Parking is $5 per vehicle. We will reimburse the parking fee at our booth for current DSPs bringing current clients.

Photo Release Agreement

I grant Essential Services LLC permission to use my likeness in photographs or video in any and all of its publications, including website entries, without payment or any other consideration. I understand and agree that these materials will become the property of Essential Services LLC and will not be returned.

I irrevocably authorize Essential Services LLC to edit, alter, copy, exhibit, publish, or distribute these photographs or videos for purposes of publicizing its programs or for any other lawful purpose. I waive the right to inspect or approve the finished product, including written or electronic content, in which my likeness appears. I also waive any right to royalties or other compensation related to the use of these materials.

I hold harmless, release, and forever discharge Essential Services LLC from all claims, liabilities, demands, and causes of action that I or my representatives, heirs, executors, or administrators may have in connection with this authorization.

I confirm that I am 18 years of age and competent to enter into this agreement. I have read and fully understand the contents, meaning, and impact of this release.

If the person covered by this agreement is under the age of 18 or otherwise unable to legally contract, a parent or guardian must give their consent to this release on their behalf.

Liability Waiver and Release Agreement

I hereby consent to participation in recreational activities described as follows.

I understand and acknowledge that I am fully aware and assume the risks (including but not limited to the risk of serious bodily injury, property loss or damage or death) of (1) participation in recreational activities and (2) my use of any associated equipment in participation in the activity(ies). I recognize my responsibility to ensure that I participate only in those activities for which I have the required skills, qualifications, training and physical conditioning. I understand that Essential Services LLC shall have no responsibility to pay for medical treatment and related costs if I am injured, or if I suffer a property loss or damage.

I assume all responsibility for all risk of bodily injury, death or property damage arising out of or related to the activity whether caused by my negligence or fault of Essential Services LLC or otherwise.

Knowing the risks described above, I agree, personally to assume all the risks and responsibilities surrounding the activity, and release, waive, discharge and covenant not to sue Essential Services LLC, its members, officers, directors, faculty, staff, volunteers, employees and agents. To the fullest extent allowed by law, I hold harmless and agree to indemnify Essential Services LLC, its members, officers, directors, faculty, staff, volunteers, employees and agents, from and against any present or future claim, cause of action, loss or liability for injury to person or property, which I may suffer or for which I may be liable to any other person related to my participation in activities resulting from any cause whatsoever, and regardless of fault.

I am at least eighteen years of age and understand and agree that no oral or written representations can or will alter the contents of this document. I agree that this agreement shall be governed by the laws of Oregon. I understand that this is a legally binding agreement and release.

If applicable, a parent or guardian may consent to the participation of a minor in these activities and agrees to all the terms, waivers, and releases described herein, both personally and on behalf of the minor.
